    The purpose of this study was to test the null hypothesis that there is no association between perioperative laboratory markers (serum albumin and hemoglobin A1c [HbA1c]) and incidence of surgical site infection (SSI) after soft tissue upper extremity surgery.

    We analyzed patient-level data from a large, insurance-based database containing supplemental laboratory results. We identified patients undergoing soft tissue upper extremity surgery (defined as carpal tunnel release, trigger finger release, wrist ganglion excision, cubital tunnel release, Dupuytren partial fasciectomy, or first dorsal compartment release) with serum albumin or HbA1c measurements within 90 days of surgery. We stratified patients into cohorts based on serum albumin concentration (<3.5 g/dL) and HbA1c (≥7%) thresholds. The primary outcome was incidence of SSI within 30 days following surgery. selleck chemical We constructed multivariable logistic regression models to adjust for patient demographics and baseline comorbidities using the Elixhauser comorbidity index.

    Patients with hypoalbuminemia experienced an SSI incidence of 3.5% compared to 0.9% in patients with normal serum albumin. In multivariable analysis, the odds ratio of SSI with hypoalbuminemia was 3.32 (95% CI, 2.32-4.65). Patients with HbA1c ≥ 7% experienced an SSI incidence of 1.1% compared to 0.7% in patients with HbA1c < 7%.     Hypoalbuminemia and elevated HbA1c (in patients with diabetes) are risk factors for SSI within 30 days following soft tissue upper extremity surgery. Preoperative measurement of these laboratory markers may be a useful tool for risk stratification and identification of high-risk patients for nutritional or glycemic optimization.

    We hypothesized that a vascularized ulnar periosteal pedicled flap (VUPPF) is a versatile graft applicable in adult patients that yields good outcomes and is a reliable alternative to other vascularized bone grafts to reduce both the technical demands and donor site morbidity of other options.

    We reviewed 11 adult patients who underwent surgical treatment of forearm atrophic nonunion with a VUPPF. Patients’ demographics, outcomes (measured by pain on the visual analog scale; Quick Disabilities of the Arm, Shoulder, and Hand score; range of motion; and grip strength), and associated complications were reported.

    Of the 11 patients, 5 had previous surgery in an attempt to treat the nonunion with an autologous cancellous bone graft from the iliac crest or olecranon. The average time from nonunion until the VUPPF was 9 months (SD, ±3 months; range, 6-14 months). The mean visual analog scale score improved considerably after surgery (8.7 vs 0.6), and considerable improvement was also noted in the Quick Disabilities of the Arm, Shoulder, and Hand score (50 vs 6). A notable improvement was seen in grip strength after surgery. Pronation/supination also improved considerably between the preoperative assessment and the final postoperative follow-up.

    Patients with cancer may be at increased risk of more severe COVID-19 disease; however, prognostic factors are not yet clearly identified. The GRAVID study aimed to describe clinical characteristics, outcomes, and predictors of poor outcome in patients with lung cancer and COVID-19.

    Prospective observational study that included medical records of patients with lung cancer and PCR-confirmed COVID-19 diagnosis across 65 Spanish hospitals. The primary endpoint was all-cause mortality; secondary endpoints were hospitalization and admission to intensive care units (ICU).

    A total of 447 patients with a mean age of 67.1 ± 9.8 years were analysed. The majority were men (74.3 %) and current/former smokers (85.7 %). NSCLC was the most frequent type of cancer (84.5 %), mainly as adenocarcinoma (51.0 %), and stage III metastatic or unresectable disease (79.2 %). Nearly 60 % of patients were receiving anticancer treatment, mostly first-line chemotherapy. Overall, 350 (78.3 %) patients were hospitalized for a mean of SARS-CoV-2.

    The duration of stereotactic body radiotherapy (SBRT) for early-stage non-small cell lung cancer (NSCLC) may affect patient outcomes. We aimed to determine the impact of a continuous versus discontinuous SBRT schedule on local control (LC) and overall survival (OS) in NSCLC patients.

    Consecutive NSCLC stage I patients (475) treated with SBRT in four centers were retrospectively analyzed. The delivered dose ranged from 48 to 75 Gy in 3-10 fractions. Based on the ratio between the treatment duration (TD) and number of fractions (n), patients were divided into two groups continuous schedule (CS) (TD ≤ 1.6n; 239 patients) and discontinuous schedule (DS) (TD > 1.6n; 236 patients). LC and OS were compared using Cox regression analyses after propensity score matching (216 pairs).

    The median follow-up period was 41 months. Multivariate analysis showed that the DS (hazard ratio (HR) 0.42; 95 % confidence interval (CI) 0.22-0.78) and number of fractions (HR 1.24; 95 % CI 1.07-1.43) were significantly associated with LC. The DS (HR 0.67; 95 % CI 0.51-0.89), age (HR 1.02; 95 % CI 1-1.03), WHO performance status (HR 2.27; 95 % CI 1.39-3.7), and T stage (HR 1.4; 95 % CI 1.03-1.87) were significantly associated with OS. The 3-year LC and OS were 92 % and 64 % and 81 % and 53 % for DS and CS treatments, respectively (p < 0.01). Cox analysis confirmed that the discontinuous SBRT schedule significantly increased LC and OS.

    HIV is an exclusion criterion for most lung cancer (LC) trials, however LC is the most common non-AIDS-defined malignancy in people living with HIV (PLHIV), poorer prognosis than the general population.
