The essay argues that the novel presents adolescent suicide as unknowable, using mythic and clinical language to distance the reader from the girls’ experience and working to mystify and decontextualise suicide. Tracing patterns of representative language through the novel, the essay shows how the sisters of the title are prevented from speaking and being heard, contributing to their suicides. The essay further argues that this representation underscores the need for adolescents to feel heard in medical contexts and that the inclusion of fiction and popular literature in medical education and training may help provide a frame to explore issues related to adolescent mental health and suicide.
A self-limited hepatitis B infection can reactivate in patients under immunosuppression or chemotherapy (reappearance of hepatitis B surface antigen (HBsAg) or HBV-DNA). Exact circumstances of HBV reactivation in patients undergoing liver transplantation (LT) for end-stage liver diseases (ESLD) unrelated to HBV are unknown, and recommendations on HBV prophylaxis remain unclear.
Among 1273 liver transplants, 168 patients with a self-limited HBV hepatitis B infection prior to LT were identified from our prospective liver transplant database. Patients with underlying chronic HBV infection and recipients of an anti-HBc-positive liver were not included in the analysis. Affinity-based purification was performed with newly synthesized disubstituted 4-aminobenzohydrazides. In the purification results, 253-fold was achieved with a yield of 56.2%. In our current report, a single-step strategy was developed and followed as an alternative to multi-step methods commonly used for the purification of PODs. Here, we present a simple and extremely high electron-deficient compound, 5,6-dicyano[2,1,3]benzothiadiazole (CNBz), as a strong electron-accepting unit to develop a sufficiently strong donor-acceptor (D-A) interaction for NIR emission. End-capping with the electron-donating triphenylamine (TPA) unit created an effective D-A-D type system, giving rise to an efficient NIR TADF emissive molecule (λem =750 nm) with a very small ΔEST of 0.06 eV. The electroluminescent device using this NIR TADF emitter exhibited an excellent performance with a high maximum radiance of 10020 mW Sr-1 m-2 , a maximum EQE of 6.57% and a peak wavelength of 712 nm.
In vivo reflectance confocal microscopy (RCM) increases diagnostic accuracy for melanoma diagnosis when combined with dermoscopy. In patients with multiple atypical nevi, a comparative dermoscopic approach improves melanoma recognition and saves unnecessary excision. The performance of a comparative approach combining dermoscopy and RCM has not been yet investigated.
To validate a comparative dermoscopic+RCM approach according to melanoma diagnostic accuracy in patients with multiple atypical nevi.
Consecutive patients undergoing RCM examination for ≥3 atypical melanocytic lesions, assessed with a dermoscopic+RCM comparative approach in a real-life setting, from July 2012 to April 2018 at a single centre, were enrolled. Dermoscopic and dermoscopic+RCM morphologic approaches were retrospectively applied by evaluating revisited 7-point checklist, RCM and Delphi scores for each enrolled lesion. Comparative dermoscopic and dermoscopic+RCM approaches were subsequently used. For each approach, lesions were classified as non-suspicious (long-term follow-up) or suspicious (short-term follow-up or excision) and diagnostic accuracy for melanoma diagnosis was assessed.
