Despite this surgical procedure being technically demanding, it is associated with improved clinical outcomes and functional capacities. Patients are also found to better tolerate the postoperative rehabilitation protocol. The load-distributing function is most critical in meniscal function, and meniscal extrusion suggests failure of this function, leading to the progression of osteoarthritis. The arthroscopic centralization technique has been developed to reduce meniscal extrusion; however, existing arthroscopic techniques sometimes fail to reduce the most extruded region, especially in cases with a medial meniscus (MM) posterior root tear, in which the most extruded region is on the posterior border of the medial collateral ligament, which is very difficult to approach. This Technical Note describes an arthroscopic technique for extrusion of the MM in which a centralization technique using knotless anchors efficiently reduces the MM extrusion at the posteromedial part and consequently restores the MM function. This technique efficiently reduces MM extrusion and restores its function, thus preventing the progression of osteoarthritis. The surgical results of shoulder multidirectional instability are not satisfactory. To address the structural and biological factors that are related to the low success rate of surgical treatment, we developed a whole glenoid reconstruction technique, which includes mainly 270° glenoid bone grafting and capsule labrum reconstruction, and glenohumeral ligament reconstruction. Our clinical experience indicates that the application of this technique can result in optimal shoulder stability. We consider the introduction of this technique will shed light on the surgical treatment of shoulder multidirectional instability. The purpose of this technique paper is to outline a minimally invasive technique using dual suspensory fixation with adjustable-loop devices for reconstruction of the superficial medial collateral ligament. The femoral fixation is performed through a limited approach at the anatomic origin of the medial collateral ligament, a socket is prepared, and the graft is docked using the adjustable-loop suspensory fixation. The tibial socket is prepared through a separate incision just distal to the pes anserine tendons and drilled medially to laterally perpendicular to the tibial shaft. The graft is tunneled and docked into the tibial tunnel using adjustable-loop cortical suspensory fixation on the far cortex. The knee is cycled through a full arc of motion and stressed in valgus to take initial creep out of the construct. The knee is placed in 30° of flexion and slight varus and final tension is applied to both the femoral and tibial side. With this technique, fixation can be completed with a minimally invasive incision and it allows the ability to tension the graft both on the femoral and tibial side to the desired level, providing a significant advantage over previously used interference screw techniques.Hip surgical techniques have evolved significantly, transitioning from open techniques to arthroscopic techniques. Hip arthroscopy has many advantages over open techniques, including reduced trauma to surrounding tissues, reduced risk of infection, and improved patient-reported outcome measures. Hip arthroscopic techniques are now commonly used for pathologies such as femoroacetabular impingement (FAI). FAI can include cam, pincer, or mixed impingement. Through hip arthroscopy, FAI may be treated with a femoroplasty and acetabuloplasty along with addressing any labral pathology that may exist. Owing to the capsule playing an integral role in hip stability, surgeons are now mindful of the initial approach and closure on completion of the intra-articular procedure. The most common approach for capsulotomy is the inside-out approach. GSK-LSD1 molecular weight However, this approach can be difficult in patients with a large pincer deformity. The authors describe an outside-in approach to arthroscopic hip capsulotomy. This capsular approach helps protect the labrum and articular cartilage while preserving capsular tissue.Massive rotator cuff tears have always been a worrisome situation to every orthopaedic surgeon. Patients’ functional demands are increasing with time, and this is why we aim to offer them the best options to preserve their quality of life. We found that using the Banana SutureLasso (Arthrex) has made this type of surgery much easier. We think that with the Banana SutureLasso (Arthrex) we have more access to the medial part of the rotator cuff, and that we can grab both the deep and superficial layers of the tendons while diminishing the risk of laceration. We combined an X-suture with a double-row fixation using a Corkscrew (Arthrex) on the humeral tuberosity and a SwiveLock (Arthrex) screw on lateral side of the humerus. We used only 3 portals with an extra parking portal. Another advantage is that we can adjust the sutures so they would be perpendicular to both ends of the tear. Our technique is simple, safe, and reproducible. The COVID-19 pandemic has affected all Sub-Saharan economies through a multitude of impact channels. The study determines the medium-term macroeconomic outcomes of the pandemic on the Kenyan economy and links the results with a detailed food security and nutrition microsimulation module. It thus evaluates the effectiveness of the adopted government measures to reduce the negative outcomes on food security and to enable economic recovery at aggregate, sectoral and household levels. Through income support measures, the food sector and food demand partially recover. However, 1.3% of households still fall below calorie intake thresholds, many of which are in rural areas. Results also indicate that the state of food security in Kenya remains vulnerable to the evolution of the pandemic abroad. In this paper, we quantify the divergence in the cost of current diets as compared to EAT Lancet recommendations at the subnational-level in India. We use primary data on food prices and household food purchases, and secondary data on food expenditures for a period of 12 months in 2018-19. The cost of the EAT Lancet dietary recommendations for rural India ranges between $3.00- $5.00 per person per day. In contrast, actual dietary intake at present is valued at around $1.00 per person per day. In order to get to the EAT Lancet recommendations individuals will have to spend nearly $1.00 per person per day more on each of meat fish poultry, dairy foods and fruits. The deficit in current diets relative to recommendations is marked by seasonal variations driven by volatility in the underlying food prices. This paper extends the evidence base for the affordability of the EAT Lancet diet to a subnational-level in India, using the most recent data on prices and expenditures, over time. We highlight the need for tracking rural markets at the subnational level, over time for their nutritional quality and ability to provide affordable, nutritious diets to the poor.
