We used this method in 4 patients who had moderate-to-severe mitral regurgitation from degenerative posterior leaflet (P2) prolapse and flail chordae. Results of intraoperative saline tests and postoperative transesophageal echocardiography revealed only mild insufficiency. One month postoperatively, echocardiograms showed trivial regurgitation in all 4 patients. We think that this simple, precise method for adjusting and stabilizing artificial chordae will be advantageous in mitral valve repair.
Population aging will pose huge challenges for healthcare systems and will require a promotion of positive attitudes towards older people and the encouragement of careers in geriatrics to attract young professionals into the field and to meet the needs of a rapidly growing number of old-aged patients. We describe the current demographic profile of hospital care use in Denmark and make projections for changes in the patient profile up to 2050.
The Danish population in 2013 (N = 5.63 million) was followed up for inpatient and emergency admissions recorded in Danish hospitals in 2013 using population-based registers. We combined age- and sex-specific hospital care use in 2013 with official population estimates to forecast the profile of hospital days up to 2050 with respect to age and sex.
The total number of hospital days per year is projected to increase by 42% between 2013 and 2050, from 4.66 to 6.72 million days. While small changes are projected for the population aged 0-69, the largest change is projthe relative contribution of persons aged 70+ will account for nearly 60% of all hospital days by 2050, being largest among men.Most mitochondrial proteins are synthesized as precursors that carry N-terminal presequences. The peculiar organization of Arg5,6 is conserved across fungi and reflects the polycistronic arginine operon in prokaryotes. MPP cleavage sites are also present in other mitochondrial fusion proteins from fungi, plants, and animals. (2020) present a new cost-effective approach to genotype copy number variants (CNVs) from large reduced-representation sequencing (RRS) data sets in nonmodel organisms, and thus to analyse sequence and structural variation jointly. They show that in American lobsters (Homarus americanus), CNVs exhibit strong population structure and several significant associations with annual variance in sea surface temperature, while SNPs fail to uncover any population structure or genotype-environment associations. Their results clearly illustrate that structural variants like CNVs can potentially store important information on differentiation and adaptive differences that cannot be retrieved from the analysis of sequence variation alone. To better understand the factors affecting the evolution of CNVs and their role in adaptation and speciation, we need to compare and synthesize data from a wide variety of species with different demographic histories and genome structure. The approach developed by Dorant et al. (2020) now allows to gain crucial knowledge on CNVs in a cost-effective way, even in species with limited genomic resources.
Familial Mediterranean fever (FMF) is a genetic disease of childhood and adulthood which is relatively rare in Germany. It is characterized by recurrent febrile attacks, peritonitis, pleuritis and arthritis. The established treatment with colchicine is effective and well-tolerated by most patients; however, some patients do not adequately respond or do not tolerate this treatment. Biologics can be considered for some of these patients. The Society for Pediatric and Adolescent Rheumatology (GKJR) and the German Society for Rheumatology (DGRh) have agreed to develop joint recommendations for this specific clinical situation.
Implementation of a systematic literature search (SLR) on the basis of the EULAR recommendations published in 2016 as the foundation for the development of evidence-based treatment recommendations for FMF patients with insufficient response or intolerance to colchicine.
The SLR was performed using references from various databases as an update of the SLR carried out by EULAR up to 2014, whereby all articles must have been published between 1 January 2015 and 31 December 2017.
