    Seizures and seizure-like electroencephalography (EEG) patterns, collectively referred to as “ictal interictal injury continuum” (IIIC) patterns, are commonly encountered in critically ill patients. Automated detection is important for patient care and to enable research. However, training accurate detectors requires a large labeled dataset. Active Learning (AL) may help select informative examples to label, but the optimal AL approach remains unclear.

    We assembled >200,000 h of EEG from 1,454 hospitalized patients. From these, we collected 9,808 labeled and 120,000 unlabeled 10-second EEG segments. Labels included 6 IIIC patterns. In each AL iteration, a Dense-Net Convolutional Neural Network (CNN) learned vector representations for EEG segments using available labels, which were used to create a 2D embedding map. Nearest-neighbor label spreading within the embedding map was used to create additional pseudo-labeled data. A second Dense-Net was trained using real- and pseudo-labels. We evaluated several strategies for selecting candidate points for experts to label next. Finally, we compared two methods for class balancing within queries standard balanced-based querying (SBBQ), and high confidence spread-based balanced querying (HCSBBQ).

    Our results show 1) Label spreading increased convergence speed for AL. 2) All query criteria produced similar results to random sampling. 3) HCSBBQ query balancing performed best. Using label spreading and HCSBBQ query balancing, we were able to train models approaching expert-level performance across all pattern categories after obtaining ∼7000 expert labels.

    Our results provide guidance regarding the use of AL to efficiently label large EEG datasets in critically ill patients.In many countries, a certain proportion of individuals living in the vicinity of wind power areas have reported symptoms that they have intuitively associated with infrasound from wind turbines. While the reason for these symptoms remains under debate, this is the first study to describe the phenomenon by assessing the prevalence and severity of these wind turbine infrasound related symptoms as well as factors associated with being symptomatic. Four wind power areas in Finland assessed to have the most problems intuitively associated with wind turbine infrasound were selected for the study. The questionnaire was mailed to 4847 adults in four distance zones (≤ 2.5 km, > 2.5-5 km, > 5-10 km, > 10-20 km from the closest wind turbine), and 28% responded. In the closest distance zone, 15% of respondents reported having symptoms that they have intuitively associated with wind turbine infrasound. In the whole study area, the symptom prevalence was 5%. Many of the symptomatic respondents were annoyed by audible wind turbine sound and associated their symptoms also with vibration or electromagnetic field from wind turbines. One third of the symptomatic respondents rated their symptoms severe, and the symptom spectrum was very broad covering several organ systems. In multivariate models, many factors such as proximity to wind turbines, impaired health status, being annoyed by different aspects of wind turbines and considering wind turbines as a health risk were associated with having wind turbine infrasound related symptoms. Although causal relationships cannot be assessed based on a cross-sectional questionnaire study, it can be speculated that interpretations of symptoms are affected by many other factors in addition to actual exposure.

    While animal data support an association between prenatal exposure to endocrine disrupting chemicals (EDCs) and altered mammary gland development and tumorigenesis, epidemiologic studies have only considered a few classes of EDCs in association with pubertal growth and development in girls. Polycyclic aromatic hydrocarbons (PAH) are a class of EDCs that have not been rigorously evaluated in terms of prenatal exposure and pubertal growth and development in girls.

    In a New York City birth cohort of Black and Hispanic girls (n=196; recruited 1998-2006), we examined associations of prenatal PAH exposure with self-reported age at growth spurt onset, breast development onset and menarche, and clinical measures of adolescent body composition including body mass index, waist-to-hip ratio, and body fat measured at ages 11-20 years.

    We measured prenatal exposure to PAH using personal air monitoring data collected from backpacks worn by mothers during the third trimester of pregnancy (data available for all 196 gih. The statistically significant associations for age at growth spurt onset and menarche were not impacted by adjustment for birthweight or childhood body size. No differences in BMI-for-age z-score, waist-to-hip ratio, or percent body fat were found between girls in the highest versus lowest tertile of ambient Σ8 PAH. Results were similar when we evaluated benzo[α]pyrene-DNA adduct levels.

Timely reperfusion is critical to limit infarct size and salvage the ischemic myocardium. However, reperfusion may exacerbate lethal tissue injury, a phenomenon known as myocardial ischemia/reperfusion (I/R) injury. Pharmacological postconditioning (PPC), a strategy involving medication administration before or during the early minutes of reperfusion, is more efficient and flexible than preconditioning or ischemic conditioning. Previous studies have shown that various mechanisms are involved in the effects of PPC. MSA-2 In this review, we summarize the relative effects and potential underlying mechanisms of PPC to provide a foundation for future research attempting to develop novel treatments against myocardial I/R injury.
