    Delayed health-seeking continues to contribute to preventable maternal and neonatal deaths in low resource countries. Some of the strategies to avoid the delay include early preparation for the birth and detection of danger signs. We aimed to assess the level of practice and factors associated with birth preparedness and complication readiness (BPCR) in Kenya and Tanzania.

    We conducted community-based multi-stage cross-sectional surveys in Kilifi and Kisii counties in Kenya and Mwanza region in Tanzania and included women who delivered two years preceding the survey (2016-2017). A woman who mentioned at least three out of five BPCR components was considered well-prepared. Bivariate and multivariable proportional odds model were used to determine the factors associated with the BPCR. The STROBE guidelines for cross-sectional studies informed the design and reporting of this study.

    Only 11.4% (59/519) and 7.6% (31/409) of women were well-prepared for birth and its complications in Kenya and Tanzania, respomplications in Kilifi, Kisii and Mwanza region. Improving level of education, creating awareness on danger signs during preconception, pregnancy, childbirth, and postpartum period, and encouraging antenatal care and skilled birth care among women and their male partners/families are recommended strategies to promote BPCR practices and contribute to improved pregnancy outcomes in women and newborns.Objective. This study aimed to understand the prevalence of sleep disorders among shift workers and analyze the relationship between sleep disorders and shift work. Methods. Baseline data were collected from subjects who were then followed for 2 years in a prospective cohort study. The cohort ultimately included 2453 people starting in May 2013, and follow-up with questionnaires was performed in July 2014 and October 2015. Sleep disorders were assessed with the Pittsburgh sleep quality index. Results. The risk of sleep disorders among two-shift workers (relative risk [RR] = 1.318, 95% confidence interval [CI] [1.025, 1.695]), three-shift workers (RR = 1.326, 95% CI [1.048, 1.679]) and four-shift workers (RR = 1.334, 95% CI [1.062, 1.675]) was higher than that among non-shift workers, and an increasing trend was observed in sleep disorders as the number of shifts increased. Conclusions. Shift workers have a higher incidence of sleep disorders than non-shift workers. An increasing linear trend was observed between the number of shifts and sleep disorders.     The role of nutrition in the pathogenesis of rheumatic diseases, including rheumatoid arthritis (RA), has gained increasing attention in recent years. A growing number of studies have focussed on the diverse nutritional contents of beverages, and their possible role in the development and progression of RA. Main body We aimed to summarise the current knowledge on the role of a range of beverages in the context of RA. Beverages have a key role within the mosaic of autoimmunity in RA and potential to alter the microbiome, leading to downstream effects on inflammatory pathways. The molecular contents of beverages, including coffee, tea, and wine, have similarly been found to interfere with immune signalling pathways, some beneficial for disease progression and others less so. Finally, we consider beverages in the context of wider dietary patterns, and how this growing body of evidence may be harnessed by the multidisciplinary team in patient management.

    While there is increasing work focussing on the role of beverages in RA, integration of discussions around diet and lifestyle in our management of patients remains sparse.
