Published by Elsevier Inc. on behalf of University of Washington.Objective The purpose of this meta-analysis was to determine whether platelet-rich plasma (PRP) was better than hyaluronic acid (HA) for the treatment of knee osteoarthritis (OA) in overweight or obese patients. Design Two reviewers independently used the keywords combined with free words to search English-based electronic databases according to Cochrane Collaboration guidelines, such as PubMed, Embase, ScienceDirect, and Cochrane library. The pooled data were analyzed using RevMan 5.3. Results Ten randomized controlled trials (RCTs) with 1096 patients were included. During the first two months of follow-up, there was no significant difference between the two groups. At the 3rd, 6th, and 12th months of follow-up, the pooled analysis showed that PRP was better than HA for the treatment of knee OA in overweight or obese patients. There were significant differences between the two groups at Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) total score (3 months MD = -1.35, [95% CI -2.19 to -0.50], P=0.002, I 2 = 0%; 6 months MD = -7.62, [95% CI -13.51 to -1.72], P=0.01, I 2 = 88%; 12 months MD = -12.11, [95% CI -20.21 to -4.01], P=0.003, I 2 = 94%). Conclusions For overweight or obese patients with knee OA, intra-articular injection of PRP in a short time was not necessarily superior to HA, but long-term use was better than HA in pain and functional relief. However, only few studies have investigated the impact of catastrophic thinking on chronic pain (CP) in the community-dwelling elderly population. This study aimed to evaluate the characteristics of CP in the community-dwelling elderly population and to investigate the effects of different periods of CP on cognitive and psychological functions. A total of 187 community-dwelling elderly people met the inclusion criteria and were included in this cross-sectional study. The survey items included demographic data (age and gender), pain-related questionnaires, psychological and cognitive functions, and sleep status. The duration of CP was investigated using three categories no pain and pain for ≤1 year and ≥1 year. A logistic regression analysis was performed to identify the factors most strongly associated with the presence of CP. The difference in each assessment was compared according to duration of CP among the three groups and analyzed using the chi-square test, Kruskal-Wallis test, and one-way analysis of variance. The PCS scores and depression scores were significantly higher in long duration of CP compared with no pain and pain for ≤1 year. The present study is consistent with the fear-avoidance model and was concluded that community-dwelling elderly people with CP are depressive and tend to magnify their pain with long duration of CP. However, there are few reports on mNGS of lung biopsy tissues for pulmonary infection diagnosis. In addition, radial endobronchial ultrasound (R-EBUS) is widely used to detect peripheral pulmonary lesions (PPLs), but it is rarely used in the diagnosis of peripheral lung infection. click here Objective The present study aims to evaluate the combined application of R-EBUS-guided transbronchial lung biopsy (TBLB) and mNGS for the diagnosis of peripheral pulmonary infectious lesions. Methods From July 2018 to April 2019, 121 patients from Tianjin Medical University General Hospital diagnosed with PPLs and lung infection were enrolled in this prospective randomized study . Once the lesion was located, either TBLB or R-EBUS-guided-TBLB was performed in randomly selected patients, and mNGS was applied for pathogen detection in lung biopsy tissues. The results of mNGS were compared between the TBLB groupuided TBLB is a safe and effective technique in the diagnosis of peripheral lung infectious lesions. R-EBUS significantly facilitates the accurate insertion of bronchoscope into the lesions, which improves positivity rate of mNGS analysis in pathogen detection. The R-EBUS probe position within lesion produced a higher positivity rate of mNGS analysis. Nevertheless, the presence of anechoic and luminant areas on ultrasonic image was correlated with poor mNGS positivity rate. Copyright © 2020 Guangsheng Li et al.Purpose To plan a treatment strategy for a spinal arteriovenous shunt (SAVS), identifying the artery of Adamkiewicz (AKA) and its origin is indispensable. However, detecting the AKA is very difficult in patients with an SAVS when using computed tomography angiography (CTA) by the usual method to find the hairpin curved artery because dilated drainage veins nearly always coexist with the hairpin curved AKA. We designed a method to identify the AKA by focusing on the diameter and pathway of the anterior radiculomedullary arteries (RMAs). Material and methods Seven consecutive patients with an SAVS were surveyed. They underwent contrast-enhanced CTA and conventional angiography from January 2009 to December 2012. Two readers evaluated the CTA images and assumed that the AKA was the artery that ran through the anterior portion of the neural foramen and continued to pass on the ventral side of the spinal cord. Results Among the seven patients, nine AKAs were detected with conventional angiography. When using our method, seven AKAs and six AKAs were identified on CTA by Reader 1 and Reader 2, respectively. The average sensitivity was 72.3%, and the specificity, accuracy, positive predictive value, and negative predictive value were sufficiently high (i.e. > 85%) for both readers. The kappa value for detecting the AKA was 0.98. Conclusions Detecting the origin of the AKA with CTA is challenging in patients with an SAVS.
