The difference in pain and quality of life aspects between obese elderly individuals with hypertension and diabetes should be accounted for in health-care programs designed for these individuals.

The consequences of acute normovolemic hemodilution (ANH) following different types of fluids on the different components of the glycocalyx and on vascular barrier permeability (VBP) remain unknown.
The aim of the study was to investigate whether the microcirculatory disruption and glycocalyx shedding induced by ANH alters VBP and whether this is affected by the composition and volume of the resuscitation fluid.
Anesthetized Wistar albino rats (n=24) underwent stepwise ANH at hematocrit levels of 35%, 25%, 20%, and 15% induced by the exchange of blood with 6% balanced hydroxyethyl starch (11), balanced crystalloid (13), and normal saline (NS) (13). Glycocalyx-shed products were measured at each level of hemodilution. VBP was reflected in the decay of fluorescence dyes of different molecular size and their plasma retention ratios. Edema was assessed by measuring organ water content and muscle microcirculation by hand-held videomicroscopy.
NS caused increased degradation of heparan sulfate and hyalurona
The results of this study provide insight into the choice of fluid for optimal perioperative fluid management and the consequences of fluid type on the vascular barrier, glycocalyx, and microcirculation.
The objective of this randomized clinical trial was to clinically evaluate the incidence of postoperative pain after root canal treatment in symptomatic irreversible pulpitis of multirooted teeth using two different instruments design rotary file systems, namely, Protaper Next (PTN) and V taper 2H (VT2H).
In this prospective randomized clinical trial, 60 patients with symptomatic irreversible pulpitis of multirooted teeth, indicated for root canal therapy, were randomly assigned to two groups according to instrument system used, namely, PTN and VT2H. Root canal treatments were performed in single visit. After treatment, the participants were asked to rate the intensity of post-operative pain on modified verbal descriptor scale after 8 h, 24 h, 48 h, and 72 h over the telephone by a second investigator. Patients were advised to call the second investigator by telephone if they felt very uncomfortable at any point of the follow-up time. At that time, they were asked to take ibuprofen 200 mg as the rescue drug.
No statistically significant difference was found among the two groups in relation to post-operative pain and intake of analgesic medication at 4 time points assessed (
>0.05, Kruskal-Wallis test).
The PTN system and VT2H rotary system were found to be equal in the incidence of post-operative pain and pain intensity was found to decrease with time, postoperatively, in both the rotary systems.
The instrument design has little effect on post-operative pain in symptomatic irreversible pulpitis patients after single-visit root canal therapy.

In this website , we systematically reviewed the distribution of anaplasmosis among domestic ruminants in Iran. Five and four English and Persian databases were studied, respectively, based on keywords and throughout 17 years (2001-2017). Thirty-eight articles were included in this systematic review and meta-analysis. Totally, 5093 cattle, 1958 sheep, and 1232 goats corresponding to prevalence of Anaplasma infection from different areas of Iran were examined. The total prevalence of Anaplasma infection was estimated to be 34% (95% CI 27%, 41%) in domestic ruminants. Based on our data, Khozestan (54%) and Khorasan Razavi (46%) provinces were the most prevalent areas in Iran and Kerman (3%) and Hamedan (1%) provinces are the lowest. The highest prevalence of Anaplasma spp. infection was belonged to A. ovis (44%) and the lowest to A. phagocytophilum (1%) with a significant difference among them (p less then .001). In addition, the most common diagnostic tests were PCR (54%), microscopy (35%) and ELISA (7%) assays. The high prevalence of ovine and bovine anaplasmosis in Iran, confirms the stability situations of animal anaplasmosis in the studied regions particularly northeastern and southwestern parts of the country. Our data offer valuable and encouraging information as regards the current situation of anaplasmosis in domestic livestock in Iran, which might be useful for active and passive surveillance and preventing plans.Cystic echinococcosis is a zoonotic disease that causes economic losses and public health problems throughout the globe. #link# Present study was undertaken to estimate the seroprevalence of cystic echinococcosis in humans of South Kashmir and to determine the risk factors associated with this disease. The present study was carried out from April 2017 to March 2018, during which 458 blood samples (from 222 males and 236 females) were collected from selected subjects from 12 villages of four districts along with demographic characteristics. Samples were analyzed by an Enzyme-linked Immunosorbent Assay (ELISA) (commercially prepared kit) for detection of immunoglobulin IgG against cystic echinococcosis. Out of 458 samples, 20(4.36%) samples were found positive. Results implied that the seroprevalence of cystic echinococcosis in South Kashmir had significant relationship with age, gender and occupation (P less then .05). Seroprevalence for cystic echinococcosis was significantly higher among males, children, and illiterate persons. In Colombia, dogs and opossum are the most important mammals in domestic and sylvatic T. cruzi transmission. However, the role of both species has not been evaluated in areas where both species converge in the peridomestic area. To evaluate the infection status of domestic and wild mammals in peridomestic habitats of Puerto Valdivia, Antioquia Department. The infection of domestic dogs and small wild mammals was performed by hemoculture, molecular and serological methods. Additionally, the infection in children under 15 years old and triatomine searches was carried out. We found that 16.07% and 34% dogs, and 59.1% and 61.1% Didelphis marsupialis were found positive by molecular and serological methods respectively. Moreover, in 25% and 75% of the infected dogs were detected TcIDom and TcI sylvatic, respectively, while all the D. marsupialis were infected with TcI. Six Rattus rattus and three Proechimys semispinosus were captured but without T. cruzi infection. Finally, none of the 82 children were positive and no triatomine bugs were captured.
