Despite the challenges faced in the public healthcare system of a developing country, outcomes of APL patients treated at our center are similar to outcomes from developed countries.
chimeric antigen receptor-modified T cell (CAR-T) therapy is an effective and promising treatment for refractory and multiply relapsed B-cell acute lymphoblastic leukemia (B-ALL). Because of its side effects and poor responses such as neurotoxicity and cytokine release syndrome, patients with central nervous system leukemia were excluded in most previous clinical trials of CAR-T treatment.
We enrolled 3 B-ALL patients with central nervous system leukemia relapse. They were infused with CD19-specific CAR-Ts, and their clinical responses were evaluated by bone marrow smear, flow cytometry, and cytogenetic alterations detected by quantitative PCR, interleukin-6, and the expansion and persistence of circulating CAR-Ts in peripheral blood and cerebrospinal fluid.
After CAR-T infusion, 2 of the 3 patients experienced bone marrow minimal residual disease-negative complete remission, and all patients tested negative for residual leukemia cells in cerebrospinal fluid tested by flow cytometry. These 3 patients experienced grade 2 or 3 cytokine release syndrome, which resolved completely after symptomatic treatment. None experienced neurotoxicity or needed further intensive care.
CAR-T infusion is a potentially effective treatment for relapsed/refractory B-ALL patients with central nervous system involvement.

Vaginal stenosis (VS) is a common side effect of pelvic radiotherapy for gynecological cancer in women. It has a high incidence variability, likely due to unstandardized and subjective assessment methods. Furthermore, even though the worldwide standard treatment for VS is vaginal dilation, low compliance rates have been noted.
To evaluate the parameters used to diagnose VS and to assess whether the lack of an objective measure of VS hampers vaginal dilator use.
A systematic review in accordance with the PRISMA reporting guidelines was conducted. PubMed, EMBASE, and Web of Science databases were searched. Randomized trials and prospective, retrospective, and cross-sectional studies published from January 2011 to February 2020 were included.
The main outcome of this study was a review of the published literature on assessment methods for VS and compliance to vaginal dilator use.
Of the 28 articles obtained, only 7 used objective methods to measure the vaginal volume. 3 studies have demonstrated patienh dropout rate during follow-up and no consensus on starting time or ideal usage. VS is frequently diagnosed using subjective parameters. Few unstandardized objective methods are used to evaluate this condition. Regarding compliance to vaginal dilator use, there was a high dropout rate during follow-up and no consensus on starting time or ideal usage. Haddad NC, Soares Brollo LC, Pinho Oliveira MA, et al. There has been a tangible increase in pornography consumption during the past decade, with the absence of a large-scale study of Arab countries.
The present study aimed to assess the prevalence of pornography viewing and its associated risk factors in Arab countries.
A large cross-sectional online survey was carried out recruiting participants without restrictions on the age, socioeconomic level, job, or educational level. Multivariable logistic regression analysis was performed to identify possible risk factors for viewing porn and results were expressed as odds ratios (ORs) and 95% confidence interval (95% CI).
Main outcomes were pornography viewing (first exposure and frequency), the perceptions of this act, use of spare time, physical activity (exercising), and frequency of psychiatrist visits.
The final number of participants included in the study was 15027 participants with a mean age ± standard deviation of 23.82 years ± 24.99. Most of the participants were men (84.56%), living with parents (ions were the cross-sectional design (cannot indicate causality) and the self-report nature (liable to social desirability and recall bias).

Patients with spinopelvic pathology, including lumbar spine stiffness and sagittal spinal deformity, are at increased risk for postoperative complications, including instability, dislocation, and revision after total hip arthroplasty (THA). Recent evidence has suggested that the Lewinnek safe zone should no longer be considered an appropriate target for all patients, especially those with spinopelvic pathology, as the safe zone is a dynamic rather than static target. There are 2 distinct issues for arthroplasty surgeons to consider lumbar spinal stiffness and sagittal spinal deformity, each of which has its own management.
In order to manage patients with spinopelvic pathology undergoing THA, a basic understanding of spinopelvic parameters, including sagittal balance, sacral slope, and anterior pelvic plane, is essential. Techniques outlined in this manuscript describe a systematic preoperative work-up and intraoperative management of acetabular component positioning according to patient-specific spinopelvic parameters, ensuring optimal component placement and a reduced risk for impingement, instability, and poor postoperative outcomes.
