Prearthritic hip disease is challenging to define, diagnose, and treat. The mechanically abnormal hip, which has not yet suffered irreversible damage but will do so if its abnormal mechanics are not corrected, can be considered to be in a prearthritic stage. A conundrum regarding the treatment of many patients with a variety of hip diseases involving a hip at mechanical risk is that the best opportunity for preventing progressive osteoarthritis lies in the period before symptoms are severe enough to demand treatment solely on that basis. It is difficult to institute treatment for a minimally symptomatic but at-risk hip because of the multitude of variables that affect symptoms and hip longevity. Input from an established team of multiple hip experts with varying areas of expertise is ideal. Shared decision-making is highly effective in this prearthritic situation of high patient sensitivity. The cornerstone of treatment of the prearthritic hip is the correction of the problematic mechanical abnormality. As methods of analysis and treatment methods evolve and as long-term outcomes of treatments become understood, the role of intervention during the prearthritic stage of hip disease is likely to increase.
The purpose of the present study was to estimate the cost-effectiveness of treating displaced, intra-articular distal radial fractures with volar locking plate fixation compared with augmented external fixation.
A cost-utility analysis was conducted alongside a randomized, clinical trial comparing 2 surgical interventions for intra-articular distal radial fractures. One hundred and sixty-six patients were allocated to either volar locking plate fixation (84 patients) or external fixation (82 patients) and were followed for 2 years. Health-related quality of life was assessed with the EuroQol-5 Dimensions and was used to calculate patients’ quality-adjusted life-years (QALYs). Resource use was identified prospectively at the patient level at all follow-up intervals. Costs were estimated with use of both a health-care perspective and a societal perspective. Results were expressed in incremental cost-effectiveness ratios, and uncertainty was assessed with use of bootstrapping methods.
The average QALY valug cost-effective, respectively. From the societal perspective, volar locking plate treatment had a 90% likelihood of being cost-effective.
External fixation was less cost-effective than volar locking plate treatment for distal radial fractures from a societal perspective, primarily because patients managed with external fixation had a longer absence from work.

HIV-infected women (WLHIV) have more than 10-fold higher risk for squamous cell cancer of the anus. Experts suggest cytology-based strategies developed for cervical cancer screening may prevent anal cancer by detecting anal cytologic or histological high-grade squamous intraepithelial lesion (hHSIL) for treatment. Currently, there is no consensus on anal-hHSIL screening strategies for WLHIV.
Between 2014 and 2016, 276 WLHIV were recruited at 12 US AIDS Malignancy Consortium clinical trials sites to evaluate hHSIL prevalence and (test) screening strategies.

Little is currently known about the surgical practices in orthopaedic emergencies in the early days of the COVID-19 outbreak (1). This is a retrospective case series of 10 orthopaedic trauma patients who underwent fracture fixation in March 2020. Of the 10 patients testing COVID-19 positive, there were a total of 16 long bone fractures, 5 pelvic ring fractures, and 1 lumbar burst fracture. There were 7 (70%) males in this cohort. Two (20%) of the COVID-positive patients did not develop fever, leukocytosis, respiratory insufficiency, or positive imaging findings and were younger (average age 25.5 years) with fewer comorbidities (average 0.5) compared with the 8 symptomatic COVID-19-positive patients (56.6 years with 1.88 comorbidities). Advancement of COVID-19 pathogenesis with lung opacities and prolonged intubation occurred in all 5 patients who remained on ventilation postoperatively (range 9 hours-11 days).
